25-Hydroxy and 1,25-Dihydroxy Vitamin D Are Not Substitute Tests

Similar vitamin D names refer to different measurements. Check the complete assay name before comparing an order or report.

The commonly used blood test for vitamin D status measures 25-hydroxyvitamin D, often written 25(OH)D. A test for 1,25-dihydroxyvitamin D measures an active form. The extra characters identify a different assay, not a premium edition of the same result.

Why two numbers may not belong on one graph

If an old report measured one form and a new report measured the other, the values do not form a simple vitamin D trend. Units and reference intervals may also differ. A spreadsheet that shortens both names to “D” discards information needed for interpretation.

Keep the full assay name in any personal record. If a provider portal abbreviates it, open the detailed report or ask the laboratory for clarification. Do not use a search engine conversion to turn one assay into the other.

Supplements do not resolve the ambiguity

An unclear result is a reason to verify the measurement and discuss it, not to increase a supplement dose. Excess vitamin D from supplements can cause harm. Tell the ordering clinician what products and amounts you take and follow their preparation guidance.

The useful first question is straightforward: “Which form was measured, and what clinical question was this test meant to answer?” Once that is clear, the report can be interpreted in the appropriate context.
